Showcasing Indonesia’s Lactation and Breastfeeding Medicine Updates: Praborini Lactation Team at ASTOT 2025, Melbourne On May 16–17, 2025, the Praborini Lactation Team from Jakarta had the honor of participating in the ASTOT 2025 Symposium, hosted by the Australasian Society for Tethered Oral Tissues at the RACV City Club in Melbourne, Australia. This international event gathered pediatricians, dentists, lactation consultants, speech therapists, midwives, and allied health professionals from across the globe to explore current knowledge and practices in addressing tethered oral tissues (TOTs), including tongue-tie, lip-tie, and buccal ties, across all ages. For the Praborini team, the symposium was not only a platform to share but also an opportunity to present uniquely Indonesian perspectives and strategies in supporting breastfeeding—especially in complex cases involving oral restrictions. As a multidisciplinary team practicing in baby-friendly hospitals and clinics, the Praborini Lactation Team contributed four scientific presentations, each grounded in real-world clinical experience and patient-centered care: The Costs of Overlooking a Tiny Tongue Tie: A Case Study of a Toddler with a Feeding Problem and Its Complex Medical Consequences—Dr. Asti Praborini, MDPaed, IBCLC, FABM A Holistic Supplementation Approach for Infants with Slow Weight Gain or Failure to Thrive Associated with Ankyloglossia—Dr. Anjar Setiani, MDPaed, IBCLC The Impact of Frenotomy on Weight Gain and Breastfeeding Outcomes in Infants with Tongue-Tie: A Case Series of Eight Patients—Dr. Agustina Santi, MDPaed, MSc, IBCLC Postpartum Depression in Mother Relating to Newborns with Tongue Tie—Dr. Iffa Rasyida, MDPaed, IBCLC Tongue Tie as a Cause of Speech Problems in Childhood—Dr. Yudith Angeline Hardjadinata, IBCLC The Role of Frenotomy and At-Breast Supplementation in Baby with Malnutrition Due to Ankyloglossia with Congenital Heart Disease—Dr. Dhika Arifa, CIMI Case Report: Successful Relactation in Twins with Tethered Oral Tissue—Dr. Ayi Dilla Septarini, MDPaed, IBCLC Successful Relactation After Frenotomy of a 6-Month-Old Baby with Tethered Oral Tissue Who Stopped Breastfeeding for 3.5 Months—Dr. Dira Firzanti (Presented by Dr. Ayi Dilla Septarini) Prevalence Frenotomy in Baby Friendly Hospital and Baby Friendly Clinics: A Descriptive Study—Dr. Andini Striratnaputri, MDPaed Postpartum Depression among Mothers of Babies with Ankyloglossia-related Breastfeeding Difficulties—Dr. Dyah Febriyanti, IBCLC, FABM Why This Participation Matters Attending ASTOT 2025 was a strategic step in amplifying Indonesia’s contribution to the global lactation and TOTs conversation. Despite the evidence-based and innovative nature of many of the team’s approaches, international forums rarely represent them. The Praborini team showed with their real clinical stories and data that managing TOTs doesn’t need to follow a one-size-fits-all Western model, but should be adjusted to fit each country’s specific culture, system challenges, and available resources. A Hallmark of Praborini Lactation Team: Cross-Disciplinary Collaboration What makes the Praborini Lactation Team unique is its truly collaborative structure—bringing together pediatricians, gynecologists, dentists, surgeons, psychiatrists, acupuncturists, lactation consultants, rehabilitation medicine specialists, midwives, nurses, and researchers. Their holistic model ensures that oral restrictions are treated not just anatomically, but also within the broader context of infant growth, maternal health, and breastfeeding success. ✈️ About ASTOT The Australasian Society for Tethered Oral Tissues (ASTOT) is a nonprofit organization that fosters interprofessional learning and dialogue about oral restrictions across the lifespan. The 2025 symposium featured sessions on oral reflexes, infant feeding biomechanics, frenotomy outcomes, and speech-language therapy integration. The Praborini Lactation Team’s presence as one of the few Southeast Asian contributors added depth to ASTOT’s core mission: embracing diverse perspectives and tailoring care based on real-world evidence from different regions. 📌 Final Thoughts The Praborini Lactation Team’s participation in ASTOT 2025 was an important step in their journey as champions for breastfeeding medicine in Indonesia. Through clinical presentations, they not only shared their experience but also built bridges between local practice and global knowledge.
